Note: This article applies to Form Creators only; Please contact your district People Admin or if you believe you should have form creator access.
You can create two different types of date fields on your forms using the calendar icon in your Doc Editor toolbar. You can chose to auto-date a field or provide the user with a calendar tool to choose their own, unique date.

How to Add an Auto-Date Field
Automatic date fields populate based on workflow activity and do not require user input.
To add an automatic date field:
1. Choose the calendar tool from the toolbar.
2. Place the date field where you would like it to appear on your form.
3. Click into the date field to open the field settings.
4. Under “How is the date added?” choose one of the automatic options.
- Automatically when submitted: Captures the date when the specific step is completed. If a form is sent back for corrections and re-submitted, the date will update to reflect the new approval date.
- Date First Initiated: Captures the first time the form moves forward in the workflow. The type is static and will not change if the form is sent back or edited later.
- Sent Date (Group Forms Only): Captures the date a group form is sent to recipients. The value remains fixed and does not change, even if responses are edited or sent back.
5. Click Save.
How to add a manual date field to your form
Manual date fields allow users to select a specific date using a calendar picker. These fields are typically used to capture information such as Date of Birth, event dates, or request-specific dates.
Here's how to add a manual date:
- Choose the calendar tool from the toolbar.

Place the date field where you would like it to appear on your form.
- Click into the date field to open the field settings panel.
- Under “How is the date added?” select “Select from Calendar”
- Mark the field as required if needed.
- Click Save.
