You do not need an Informed K12 account to submit or access forms on the Informed K12 Platform, however, some forms may require that you verify you email address.
To verify your email address:
- Access the link to your form that your district provided and enter your name and email address.
- After clicking, Go to Form, a verification code will be sent via email.
- Copy and paste the code into your form and click Verify.
- You will be taken to your blank form to fill out and submit.
