If you are a case manager who designated the route to a form and need to send it back, you can make this request through the Support widget below and we can assist! Simply provide us the name of the form, the incorrect email address and the updated email. Otherwise, you will need to contact someone at your district who has Form Manager access to the form. If you're not sure who that is, or whether you have Form Manager access, please contact Support as well.
